Wildwood Family Clinic has two clinics providing quality, personalized care for Madison and
 eastern Dane county.

Clinic Administrator: Ms. Sheryl Green, who joined the Wildwood Family Clinic family in 2003, is the administrator for both clinics.  You are always welcome to address any clinic issues with her or can use the feedback form on the "Feedback" page to e-mail your concerns to Ms. Green, our physicians and staff.

Insurances accepted:  DeanCare, Physicians Plus, WPS, WEA, Cigna, Blue Cross/Blue Shield and many others.  If your insurance provider is not listed, and you would like to know if we can provide your medical care, please call our business office at 221-1501 during regular business hours or contact your plan's customer service representative.  At the present time, we do not see patients with Unity or GHC insurance.

MADISON OFFICE:

Our Madison Office is located at 4901 Cottage Grove Road, Madison, WI  53716 and offers the full range of Family Practice services including pediatric, adult and geriatric services as well as lab, x-ray and physical therapy.  We also have mammogram and bone density measurement services at this location.  You will find interactive maps of our locations on the "How to find us" page.  Otherwise, feel free to call us at (608) 221-1501 during business hours for directions.  Providers located at the Madison Office include:

COTTAGE GROVE OFFICE:

Our new Cottage Grove Office is located at 251 E. Cottage Grove Road, Cottage Grove, WI  53527.  This clinic opened on May 24, 2010 and replaced our previous Cottage Grove office on Southing Grange.  The full range of Family Practice services is also provided at this location.  Our Physical Therapy department is located here as well as the Madison Office.  Please click here for further Physical Therapy information.  You may also find the interactive maps located on the "How to find us" page to be helpful.  Physicians located at the Cottage Grove Office include:
